Records Request Form from Previous School: PDF / Word

To start the school admissions process please contact your company’s HR Office to complete the proper forms (ask for the Educational Checklist).

Once you have completed the paperwork for HR and you have been approved for admission to Kwajalein Schools, please ensure you travel with the following documentation.

Enrollment Records Checklist

Please bring the records and information listed below when coming to the school to enroll a student.  You may also request that the school you are leaving send this information prior to your arrival (see the records request form above).

P     Birth Certificate

P     Immunization Records

P     Copy of physical given for you to get on-island or from previous school

P     Transcripts and exit grades from previous school.

P     Name, Address & Fax # of your previous school (in case we need to request additional records)

 

In order to make your family’s transition to Kwajalein Jr./Sr. High School as smooth as possible please contact the Guidance Office at (805) 355-2011 prior to your arrival and inform us of the date you would like your students to begin.
